Former congres member Barbara Jordon from Texas addressed the issue of change in her speech to the Democratic National Convention of 1992 when she said:

“YES, it is time to change.
But, I ask you, change FROM what TO what?”
and she repeated this for impact

          “CHANGE  FROM  WHAT TO WHAT?"

Why do so many change efforts fail in spite of good intentions? What are the most important keys to address from what to what? We miss one of the most important keys,  the understanding of the human dynamics in what it takes to change "FROM what TO what". The insights provide focus and direction and take into account cultural differences and values of people, teams and organisations.
Change is what we do, transformation is a different way of seeing. The starting point is naming reality and understand where people, teams and organisations are. Then see with different eyes and align to perform.
There are two approaches that are very integral and complete.
